About Three Oaks Foundation
Facing both a terminal illness and economic difficulty can add unbearable stress to an already difficult and heartbreaking situation. At the Three Oaks Foundation, we are committed to serving those in need and we rely on the generosity of our community to realize our mission.
Established in 2020, the Three Oaks Foundation is registered as a 501(c)(3) non-profit corporation whose continuing mission is to support Three Oaks Hospice patients and their families in need.
Donations received will provide for Patient Care and Family Support in addition to other expenses as approved by the Foundation’s Governing Committee.

Contributions to the Three Oaks Foundation are tax-deductible to the extent permitted by law. The Three Oaks’ tax identification number is 85-1267324. Donations are fully tax deductible and a donation receipt letter will be sent for your records.
