Hospice Care
Our well-trained nurses, certified nursing aides, chaplains, counselors, and volunteers work together to help ease symptoms, manage pain, and assist with everyday activities. Three Oaks Hospice also offers bereavement support for families, and hospice services are available in your home, in a relative’s home, or at an assisted living facility, so you or a loved one can remain in a comfortable and familiar environment.

Palliative Care in West Houston
Three Oaks Hospice palliative care services help patients and their families to manage symptoms related to the diagnosis of a chronic, incurable illness. Such symptoms can be physical, emotional, spiritual, social or financial.
Three Oaks Hospice palliative care services aim to improve the quality of life for patients with advanced illnesses. Services may include palliative medicine consultation or treatment, psycho-social assessment and intervention, spiritual counseling or therapy, palliative nursing diagnosis and interventions related to symptom management for physical signs and symptoms.
